What We Do:
Assessment, planning for services, monitoring and evaluation are provided for homebound elderly or physically disabled individuals to help them remain in their own home as long as reasonably possible.

Services Authorized:

Home Care
Including housekeeping, laundry, personal care, nursing, assistance with bathing, respite for caretakers.

Meals
Home delivered, frozen meals.

Adult Day Health
Social, recreational, and therapeutic activities and meals for individuals at a care center. Transportation is provided.

Home Repair and Adaptations
Minor repairs to client-owned homes to enable safety and utility.

Counseling
In-home professional counseling for problems in everyday living.

Senior Companions
Companions visit homebound elderly on a weekly basis.

Shopping
Shopping for homebound individuals every other week.
