How can I become a patient at St. Elizabeth’s Health Center?
- • If you are or think you may be pregnant and need access to prenatal care, call (520) 628-7871 and ask for Registration. They will arrange an appointment to assist with setting up your care and eligibility.
- • If you are seeking medical or dental care, eligibility and registration can be completed by coming to St Elizabeth’s main location between 8am-11:30am or 1:00pm-4:30pm Monday through Friday. These services are completed on a walk-in basis and no appointment is necessary
What do I need to bring to register for medical or dental care?The following (current) information is required in order to complete your interview:
- • Proof of address (utility bill such as electric bill, water bill, gas bill with your name on it) past 30 days
- • Picture ID
- • Unearned Income (Retirement, Pension Pay, or Award letter for Social Security) for all members of family past 30 days
- • Earned Income (Wages, all check stubs for prior and current month, if self-employed write statement for work expenses and gross income and please bring last year’s income tax return) for all member of family. past 30 days
- • Social Security Cards or Social Security Numbers for those applying
- • Proof of Citizenship or Alien Status (if applicable).
- • Copies of Health Insurance cards, both sides or the card itself
- • Daycare Expenses.
Based upon your individual information, you will be screened for eligibility for Kids Care, AHCCCS and PCAP as well as sliding scale medical and dental care. Your patient registration is renewed annually or as you provide new information about your income or insurance status.
St. Elizabeth’s also provides care to low income Medicare patients as well as Mercy Care and Pima Health Systems patients.
