Registration and Eligibility for Services at St. Elizabeth's
For medical and/or dental care, first time patients must register in-person at the clinic.  A sign-in sheet can be found in the lobby and the hours for registration are 8am - 11am and 1pm - 4pm, Monday through Friday.  No appointment is necessary for registration.  There is a $10.00 registration fee per person, or $25.00 per family.  This fee is due at the time of registration.  
 
If you wish to come to St. Elizabeth's for your pregnant and newborn care, call (520) 628-7871 and ask for the Registration Department.  That department will schedule an appointment to assist you with registration and eligibility.
 
The following (current) information is required for registration at St. Elizabeth's:  
  1. Proof of address (electricity bill, water bill, gas bill, etc., with your name and address on it) within the past 30 days
  2. A picture ID
  3. Proof of income that is not from work (Retirement, Pension Pay, Disability, Social Security) for all members of the family within the past 30 days
  4. Proof of work income (current and a prior month's check stub or payroll record and if self-employed, bring last year's income tax return) for all family members
St. Elizabeth's staff will assist with eligibility and enrollment for KidsCare, AHCCCS and PCAP if patients and/or family qualify for any of these programs.
 
Patient registration information will be used to determine their sliding fee scale for medical and dental services for patients with no insurance or little insurance.  Patient registration information is updated annually or if new information is provided.
 
St. Elizabeth's Health Center provides care to AHCCCS patients that are enrolled in Mercy Care Health Plan, University Family Care Health Plan, and Pima Health Systems Health Plan.
