Community Engagement Navigator

Position Purpose

Will work with individuals, medical and emergency care agencies, local governmental entities, and the community at large to seek out and engage community members who are in need of behavioral and integrated health services. Responsible for helping individuals obtain entitlements and needed services. This position is dedicated to the Tucson Homeless Work Program 3 -4 days per week.  Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

Essential Functions

  • Meet with community members face to face and educate them about entitlements and Behavioral Health Services
  • Spend at least 75% of time out of the office meeting with community members and stakeholders in the community and people’s homes
  • Respond to requests from the community, care agencies, and health plans to engage people who may need behavioral or integrated health services
  • Educate and inform community members and organizations about services
  • Establish and maintain positive working relationships with all possible referral sources
  • Maintain accurate information regarding entitlements and resource and referral information to distribute
  • Coordinate access to behavioral health services for individuals such as assisting in the agency intake process, accompany them to appointments, and provide ongoing encouragement
  • Coordinate access to entitlements through Health-e Arizona
  • Establish and maintain positive, productive working relationships in the community
  • Develop nontraditional outreach efforts to engage people in the community such as establishing relationships with the business community, medical community and government agencies; including, post offices, Social Security, National Parks and others
  • Use Motivational Interviewing techniques to engage community members into care
  • Assist with enrollment into the Homeless Management Information System (HMIS) for coordinated entry of Housing Programs
  • Coordinate with community partners to provide navigation services under the Homeless Work Program
  • Complete outreach to individuals for reengagement of behavioral and/or integrated health services
  • Complete data entry and tracking in compliance with monthly deliverables
  • May drive agency or personal vehicle on company business

Minimum Qualifications

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • AA degree
  • 3 years’ experience in Behavioral Health or Health Care
  • Must be at least 21 years of age
  • Proven skills utilizing Microsoft Office Suite
  • Excellent organizational skills
  • Proven strong communication skills – oral and written communications are clear, concise, and organized using appropriate style, grammar and tone
  • Ability to obtain State of Arizona Level One Fingerprint Clearance Card
  • Valid Arizona Driver License, proof of insurance, Arizona Motor Vehicle Division 39 month Motor Vehicle Report, and reliable, safe transportation
  • Current CPR and First Aid certification, or obtain within 30 days of employment
  • Current Mental Health First Aid certification or obtain within 30 days of employment

 

Desired Qualifications

  • Bachelor’s Degree
  • Additional years of directly related experience
  • Experience in public speaking, teaching and training
  • Bilingual English/Spanish verbal and written
