Daybreak is a licensed medical facility with a
licensed nurse providing care and attending to medical needs on site. Each
participant is admitted to the program with a physician’s order.
All of our staff is CPR and First Aid certified and trained in the specialized care services provided by the program to meet the unique needs of the participants.
We provide nutritious snacks and meals that are prepared on site by certified staff and comply with CACFP standards.
Our days are filled with fun activities and exercises that promote mobility, brain activity and social skills. All activities are offered in English and Spanish for our participants to enjoy.
We work closely with each participant, their caregivers and their physician to develop a plan of care to provide the best medical and non-medical quality care and meet the individual needs of each participant. We also provide support for families.
Less than 5hrs per day $12.50/hr
5hrs per day $45.00/day
More than 5hrs per day $65.00/day
