Catholic Community Services in Western Arizona (CCSWA) Case Managers are professionals adhering to uncompromising Case Management standards. Case Managers serve as the link between clients and direct service providers. These services are a pivotal component for helping clients realize their maximum independence.Case Management responsibilities include: Assessing needs/eligibility Connecting clients with services Authorizing services Evaluating services Advocating CCSWA Case Management Services are targeted for seniors (60+) and adults with disabilities. CCSWA case managers spent 5,742 hours with seniors or disabled individuals to understand their needs and arrange agency and community services for them. More than 95% of funding comes from federal, state or local government. The remainder is provided by generous donations from our community. All funding, public and private, serves the local community of Yuma and La Paz Funding to support this program was provided in part or whole by the Administration of Aging and the Department of Economic Security, Aging & Adult Administration and WACOG / Area Agency on Aging.
